Re: Access to personal information request

Dear Mr. Crozier,

This is a followup to my PIPEDA request of Wed, 22 Sep 2004.

That request was for copies of any surveillance data and
reports that may pertain to me, from the afternoon of Tuesday,
September 21, 2004, as well as from the afternoon of Wednesday,
September 15th, 2004, in accordance with PIPEDA's requirements
on the disclosure of surveillance data to the surveilled.

This is a request for access to my personal information pursuant to the
Personal Information Protection and Electronic Documents Act (PIPEDA). It
is addressed to you because a frontline telephone agent for Intelligarde
said your company does not have a designated individual who deals with
access to information requests. Please forward this request to the
appropriate individual should there be someone else who handles such
requests.

Background: On the afternoon of September 15, 2004 my 2 year old daughter
and I were asked to leave the Intelligarde surveillance area of Dundas
Square in downtown Toronto. Subsequently I was part of a group that
was also asked to leave on the afternoon of Tuesday September 21, 2004,
when we were taking pictures of one of the fountains in Dundas Square.

I wish to obtaie copies of any surveillance data and reports that may
pertain to me, from this afternoon, Tues. September 21, as well as
from the afternoon of Wednesday, September 15th, in accordance with
PIPEDA's requirements on the disclosure of surveillance data to the
surveilled.

Request: I am requesting access to my personal information held by your
company at minimal or no cost to me, including any surveillance tapes,
surveillance video data, other data, or reports pertaining to me relating
to the Sept. 15th and Sept. 21 incidents. In addition to accessing my
personal information, I would like to know how you received it, an
account of how it is being used and how it has been used in the past.
I also would like to have a list of the third parties my personal
information has been given to.

Intelligarde's response requirements under PIPEDA: The Act states that an
organization must respond to requests with due diligence and no later than
thirty days after receiving the request.

Privacy: In making this request, I am citing the protection of the PIPEDA
which states that the personal information I am providing in this request
can only be used for the purposes of this request and can not be provided
to third parties without my knowledge or consent.

Sincerely,

Steve Mann
